After attending the group session, a copy of your group session report will be sent to your otolaryngologist, family physician and yourself.
All requests for patient health records (hearing tests, questionnaires, etc.) must be made in writing to the hospital's Records Management department directly. The Tinnitus Clinic and Department of Audiology are not able to provide patients, lawyers, or third-party payers with patient health records directly.
